It will review key IDG elements, including care planning, team communication, patient documentation, continuous assessment, coordination of care, and scope of services. The recommended best practices to conduct an effective IDG will be outlined to provide a structured, scheduled, and formatted IDG that meets the hospice requirements, including supporting patient eligibility. MEET THE PRESENTER: Kathy Ahearn, RN, BSN, PHN, ALECC, Inc.

Kathy Ahearn has dual bachelor’s degrees in nursing and social work. She is currently CEO and founder of ALECC, Inc., which provides consulting, mentoring, and education to all post-acute care providers, with an emphasis in the hospice industry. She began healthcare work due to a disabled parent and later a child living with a chronic disease. Her passion is healthcare, experiencing it both professionally and personally.

Kathy has always been a trailblazer in healthcare. She helped develop pain scales to standardize assessment with the American Pain Society and the Joint Commission. She expanded into medical devices, developing one of the first real-time cloud-based disease-specific software programs to assist patients, clinicians, and managed-care organizations to improve outcomes and collect data for the diabetic population. Currently, she has branched into YouTube to create a channel that will support colleagues and clinicians moving forward in healthcare.
