*KEYNOTE SPEAKER: Tracy Gendron, MS, PhD, “Ageism Unmasked

          Dr. Tracey Gendron serves as Chair for the Virginia Commonwealth University Department of Gerontology, as Director for the Virginia Center on Aging and is the author of the book Ageism Unmasked: Exploring Age Bias and How to End It. With over 25 years of experience as a grant- funded researcher and nationally recognized speaker, Tracey is dedicated to raising awareness and ending ageism through education. Tracey has a Master’s degree in Gerontology, a Master’s degree in Psychology, and a Ph.D. in Developmental Psychology.

          The 2025 HHVBP Discharge Function Score relies on OASIS GG items rather and impacts a whopping 20% of the Total Performance Score.  Although home health agencies have had GG0130 and GG0170 OASIS items for five years, clinicians and clinical leadership staff often remain unable to accurately assess and score (code) the GG items.  This session will briefly overview the Discharge Function Score calculation and then deep dive into guidance and tips for accuracy coding the GG items.  Meaningful, real-world scenarios and attention to the most misunderstood aspects of the GG items are included to make this information actionable immediately.

          This presentation describes trends in the workforce of healthcare professionals in the home care and hospice fields.  This presentation also showcases the demographics, employment factors (e.g. FTE's, geographic distribution, job satisfaction) and future plans of this workforce.  The data presented has implications for workforce planning in Virginia's home care and hospice fields.

 

* Forewarned is Forearmed- Know Before They Go! The Act of Keeping Clinicians Safe in Patients Homes, Arlene Maxim, RN, HCS-C, Axxess and Officer Mike Manis, SWAT Team Educator, Gallatin Police Dept., Gallatin, TN

          Healthcare workers face increasing violence, particularly in homecare and hospice settings, where safety measures are limited.  Workforce shortages are driven by safety concerns.  To mitigate risks, organizations must prioritize staff safety through education, pre-visit information gathering, and self-defense training.  Ensuring clinician safety improves care quality and meets ethical responsibilities.

          This session will focus on providing home health leaders with a better understanding of the Home Health Value-Based Purchasing model.  We will discuss the implications of the release of the first Annual Performance Reports and what that means for the future revenue impact.  There will also be a focus on the new scoring for Discharge to Community, replacement of the TNC measures with Discharge Function Score and now focusing on Potentially Preventable Hospitalizations.  Attendees will gain practical insights to better navigate these updates and optimize their financial and operational performance.

 

* Palliative Care Programs: Where to Start and How to Maintain, Rachel Carey, Counsel, Whiteford, Taylor & Preston, LLP

          This session will go into how to establish standalone palliative programs by addressing essential program elements, different program models, care setting and organizational structure.  For the different models, we will go through the different regulatory and license considerations such as corporate practice of medicine issues, licensing requirements for new entities and individual practitioners and scope of practice considerations.  Enrollment and credentialing payor considerations such as Medicare enrollment, Medicaid enrollment and possibly touch on commercial payor enrollment.  The session will also go over federal and state fraud and abuse considerations when setting up a new palliative program to supplement existing business.  Finally, we will discuss recent news and trends on palliative care programs and the Centers for Medicare and Medicaid Services (CMS) and Virginia Department of Medicaid Assistance Services (DMAS) to promote increased opportunities for reimbursement for these services.
